Specialized filler injection
Filler is derived from the English word fill, which is also known as gel in Iranian colloquial language. Although gel is a type of filler, anything that is injected under the skin or into the layers of the skin and causes filling is defined as a filler, regardless of its durability.
- Ultra-small molecule fillers
- Micromolecular fillers
- Macromolecular fillers
- Modify Volume
- Lift and contour
- Rejuvenation
Dr. Karimi's tips about fillers:
Fillers are divided into two categories: absorbable and non-absorbable, or permanent and non-permanent. The majority of fillers currently used in the world are absorbable fillers, and 80 to 90 percent of these fillers are hyaluronic acid fillers.
After hyaluronic acid fillers, other fillers that are used are polylactic acid, which is also absorbable, and polycaprolactone fillers, which are also absorbable. Calcium hydroxide apatite are fillers that are more durable than hyaluronic acid, and all of them are absorbable.
The most important non-absorbable fillers that have caused many crises in the past are PAAG and silicone is also a non-absorbable filler.
Nowadays, non-absorbable fillers are mainly used for body contouring, such as silicone breast implants or silicone buttock implants. These fillers are also used for leg implants and breast implants in men.
Hyaluronic acid fillers, which are the most important group of fillers and the subject of discussion, usually have several types of fillers, with reputable brands: macromolecular, medium molecular, micromolecular and ultra-micromolecular fillers. For example, in the case of Innocence filler, we have the macromolecular Shape filler. A lower level is style, a lower level is smile, and a lower level is silk. We use shape for volume. In fact, macromolecular fillers are used for volume. Most lines have macromolecular, midmolecular, and micromolecular fillers.
The most specialized filler
The most specialized fillers among fillers are ultra-fine molecule fillers, which are used for very fine lines around the eyes, forehead, around the mouth, and under the eyes.
The best fillers
In general, the best fillers are those that do not have a long shelf life and have the ability to dissolve or disappear and have minimal side effects. If we look at fillers from this perspective, the best filler is hyaluronic acid, which is 85% of fillers in the world. The fillers that are currently mainly used are: hyaluronic acid fillers such as Juvederm, Vivace, Perfecta; and fillers that are available in the Iranian market: Beltro, Revance, Innocence. These are hyaluronic acid fillers that can contain or not contain lidocaine, and it doesn’t matter.
Filler applications
Since fillers are made of hyaluronic acid and hyaluronic acid is naturally present in the tissue, in addition to correcting volume, contour, and lift, they also cause rejuvenation. In general, the main function of fillers is to correct volume, hydrate, rejuvenate by stimulating the production of collagen and elastin, and correct the contour.
General rule for using filler
The larger the molecular weight of the filler, the greater its water-holding capacity. As a result, it is used for areas where we want to correct volume. The general rule for using filler is to inject less than necessary, because fillers are hydrophilic and their molecular behavior can vary from person to person. As a result, about 30 to 40 percent less filler should be injected than is needed in the area. Especially in the temples, tear troughs, smile lines, and chin. Too much filler in the chin can be problematic due to its high movement.
Effects of fillers
The most important complication of filler injection is pressure on the vessels, which can be very dangerous. As a result, understanding the anatomy when injecting filler is very important, and for this reason, two or three principles must be observed when injecting filler: One, use a fine needle. Two, apply less pressure when injecting. Three, move the injection point frequently and use the least amount of volume in the injected area.
In addition to vascular complications, another issue with filler injections is the possibility of filler displacement. If the filler is not injected in the right location, for example, under the eye filler is injected into the fat under the eye and is not restrained by the periorbital muscle, the filler can become superficial and create an unsightly appearance. In the case of the temples, too much filler or improper injection can cause the filler to shift. The same is true for other areas.
One of the problems with fillers in Iran
One of the problems currently facing the Iranian market regarding fillers is the use of bulk fillers or 10cc fillers in the facial area, which causes many problems. Due to their long-lasting nature, these fillers, especially in the area around the mouth and cheeks, give the face an unsightly appearance: The lips become droopy and the so-called duck or camel lips become prominent, and the cheeks become prominent, which, with facial analysis, will give the appearance of squirrel cheeks. As a result, it is recommended to always use branded fillers, even in smaller amounts, and with the correct technique.
